If you have been involved in a motor vehicle accident in which a person is injured or damage to any one vehicle or property is in excess of $1000.00 you are required to complete a Massachusetts Motor Vehicle Crash Operator Report. Motor Vehicle Crash Operator Report copies are also available at any police station or Massachusetts RMV branch.
You will need to make three copies of your completed form, one for local police where the ACCIDENT OCCURRED, one copy for the Massachusetts RMV and one copy for your insurance company. Please note that if the accident occurred on a State roadway (including but not limited to: Route 93, Fellsway, Fellsway West and Route 16) you should submit the completed form to the Massachusetts State Police. You have five business days to complete and file these forms.
If you would like a copy of the Officer’s Accident Report you may request this form in writing, or made in person, by postal mail, by fax, or through electronic mail.
